Northern Care Alliance NHS Foundation Trust in Salford is seeking a highly capable GP to join a multidisciplinary team delivering care to older adults in care homes. You will provide prescribing, care planning and comprehensive assessments, working with geriatricians and allied staff to optimise patient outcomes.

Applicants must be GMC registered with CCT in General Practice and be on the NHS performers list or eligible.

Formación

• Full GMC registration with licence to practise is required.
• CCT in General Practice is essential.
• Inclusion on GP Performers List or eligibility is required.
• Experience with EMIS and Microsoft Office is advantageous.

Responsabilidades

• Prescribe repeat medications and process patient test results.
• Record clear IT-based consultation notes to agreed standards.
• Support admission avoidance with advanced care planning.
• Participate in governance, audits and QoF/ES schemes.
• Work in a multi-disciplinary team and with families on care planning.
