Job summary

Kent Community Health NHS Foundation Trust is seeking an experienced Clinical Pharmacist to join the Integrated Pharmacy Team (IPT). You will optimise medicines for patients referred to IPT and provide pharmaceutical advice across specialist services.

You will develop expertise in medicines optimisation for frailty, working with multidisciplinary teams to achieve optimal patient outcomes in community settings.

Responsibilities

• Optimising medicines choice through medication review for patients in their own home or community hospital setting
• Ensuring safe transfer of care into community hospitals including medicines reconciliation
• Participation in KCHFT and primary care network (PCN) multidisciplinary team patient reviews
• Assessment of concordance and implementation of appropriate interventions
• Providing responses to medication queries
• Provision of legal and ethical advice as appropriate for the service provision
• Undertaking clinical development and contributing to the development of others
• Supporting training to staff within the pharmacy service
• Where required undertake training to become and independent prescriber
• Supporting and facilitating Quality Improvement (QI)
• Liaising with key stakeholders in order to explore identified risk areas
