Job summary
Croydon Health Services NHS Trust is seeking a Band 6 District Nurse to join the Parkway Health Centre team providing care in patients' homes and within integrated care pathways. You will hold a district caseload within our Integrated Community Networks, delivering high quality, evidence-based nursing care.
The role requires a Registered General Nurse-Adult with a Specialist Practice Qualification or substantial community nursing experience, plus mentorship qualification and driving licence.
Qualifications
- Registered General Nurse-Adult.
- Specialist Qualification in area of Practice or significant experience in nursing adults in the community.
- Mentorship qualification.
- Nurse Prescribing or willingness to undertake
- Live registration as a sign off mentor.
Responsibilities
- Provide expert, patient-centred nursing care to patients in their homes.
- Supervise delivery of care by clinical staff within Integrated Care Teams.
- Offer education and support to patients, carers and other health care professionals.
- Work with tissue viability team to prevent and manage pressure ulcers in line with guidelines.
