Exciting Opportunity: Join Our Care at Home Team in Inverness!
We are looking for a dedicated professional to join our Care at Home team in Inverness. In this vital role, you will support the Inverness District by helping to prevent hospital admissions, enable early discharges, and coordinate care packages with independent providers as well as our in-house Enablement team.
Working closely with a multi-disciplinary team, you will focus on maximising individuals' independence, assessing their ability to remain in their own homes, and providing essential support. The post holder will independently manage a designated caseload and work autonomously on non-complex cases, with access to advice and support as needed. You will collaborate closely and be line managed by the Enablement Officer. There will be supervisory responsibility within this role.
