As Care Coordinator, you will assess, plan and coordinate care, supervise junior colleagues, and work with MDT professionals to promote safe, person‑centred services. DBS and safeguarding duties apply.

Qualifications

• Registration Mental Health Nurse RMN Part 3.
• Evidence of CPD post-qualification.

Responsibilities

• Assess and coordinate care as Care Coordinator within a defined catchment.
• Plan and implement evidence-based care with MDT support.
• Provide professional advice and supervise junior staff.
• Coordinate CPA caseload and facilitate referrals to crisis teams or inpatient care.
