We are seeking a Band 6 Community Mental Health Practitioner to join the North East Essex Dementia Service within EPUT. The role covers memory assessment, dementia care, and DIST/DST teams in community settings, focusing on proactive management and admission avoidance.

The post involves working across multiple teams with a commitment to high-quality dementia care, collaboration with health and social care partners, and delivering services that help people live independently at home.

Qualifications

• Good oral communication skills and experience in mental health community settings.
• Registration as RMN with current practicing status and CPD evidence.

Responsibilities

• Deliver memory assessment and dementia care across DIST/DST teams in the community.
• Collaborate with hospital, primary care, social care and third sector partners.
• Support individuals to live independently with holistic assessments and care planning.
