Job summary

Northumberland, Tyne and Wear NHS Foundation Trust in Newcastle is seeking a Band 6 practitioner to support older adults (55+) with substance misuse within Newcastle Treatment & Recovery (NTaR). The role is full time, based at Plummer Court, Newcastle, and focuses on assessment, management of complex needs and recovery-focused care planning.

Qualifications

• Assessment and management of caseloads of adult service users with substantial and complex needs.
• Provide specialist clinical advice to service users, carers, staff and partner agencies.
• Deliver recovery and wellbeing focused care plans with regular reviews.

Responsibilities

• Deliver evidence based clinical and psychologically based interventions.
• Coordinate treatment planning, interventions, reviews and discharge planning.
• Provide clinical supervision to staff as delegated by pathway leads.
• Work within a multi-disciplinary team to ensure high quality, individualized care.
