Job summary

NHS Jobs is seeking a Community Specialist Physiotherapist for Northwich. You will manage a clinical caseload, perform comprehensive assessments and implement treatment plans for complex cases, while upholding privacy, dignity and individual needs.

You will deliver both individual and group therapies, maintain high-quality documentation and collaborate with the MDT to ensure effective service delivery and continence of care.

Qualifications

• HCPC registration and licence to practice.
• Experience in rehabilitation and complex case management is desirable.
• Ability to work within national guidelines and legislation affecting health and social care.

Responsibilities

• Manage a clinical caseload with autonomy and work without direct supervision.
• Undertake comprehensive physiotherapy assessments and implement treatment plans.
• Provide individual or group therapy as appropriate to setting.
• Record, maintain and store accurate documentation in line with policy and local requirements.
• Communicate outcomes to relevant disciplines and contribute to MDT activities.
