Mid Cheshire Hospitals NHS Foundation Trust in Northwich is seeking a Community Specialist Physiotherapist to join the team. You will assess, manage and treat a community caseload of patients with complex needs, often working autonomously with MDT and peer support across sites.

The post is permanent and you will work with a diverse caseload including orthopaedics, falls, mobility and neurological conditions, supervising junior staff and students as part of the integrated care team.

Qualifikationen

• Ability to manage a clinical caseload with autonomy.

• Provide comprehensive physiotherapy assessment and treatment to a complex caseload.
• Develop and implement individual care plans with clear discharge objectives.
• Record, store and communicate outcomes and progress to other professionals.
• Supervise junior staff and students while maintaining professional standards (HCPC).
• Work within safeguarding, consent and legal frameworks; maintain patient dignity and confidentiality.
• Use digital tools and Microsoft Office to support documentation and remote working where appropriate.
