Job summary

Southern Hospice Group requires a clinician to provide holistic palliative care for children and young people with life-limiting conditions. You will assess needs, educate families, and work within a community team across home, community, hospices and acute settings.

Flexibility includes on-call overnight and weekends within contracted hours, with a predominantly Monday–Friday 9–5 core schedule. Benefits include pension, generous leave, study support and healthcare options.

Qualifications

• Provide holistic care to CYP with life-limiting conditions.
• Undertake holistic assessments of child and family needs.
• Maintain high quality palliative care and clear communication.

Responsibilities

• Be first point of contact for families after acceptance to the service.
• Work with in-house teams to ensure seamless care for CYP and families.
• Contribute to community, home, hospice and acute setting care as required.
