Hywel Dda University Health Board is seeking an experienced nurse leader to join the Integrated Community Team in Pembrokeshire. You will be accountable for managing nursing caseload, leading a team of community nurses, and delivering high-quality, person-centred care in patients' homes and communities, including working across North and South Pembrokeshire Localities to meet service needs.

Qualificações

• Registered Nurse with Specialist Practice Award in District Nursing (postgraduate) and eligible to pursue a Masters.
• Experience leading a community nursing team and delivering 7-day community nursing care.
• Willingness to travel between sites and work flexibly.
