This is a community-based role, visiting patients at home. A valid driving license and access to a vehicle are essential.
As an expert in your field with extended competencies (e.g., vacuum-assisted closure & syringe driver management), you will demonstrate excellent communication skills for holistic patient assessments. We support a Health Coaching approach to enhance self-management, especially for long-term conditions.
You will work within a referral centre, utilizing critical thinking and negotiation skills for decision-making and triage. You will manage a caseload of patients with delegated responsibilities to your Band 5 nursing team, ensuring continuity of care. Experience with electronic patient records (e.g., Systm One) is advantageous but training will be provided.
Understanding of collaborative working with services like ICT, GPs, Rapid Response, and Complex Care at Home is essential to ensure timely, appropriate care.

You must hold current NMC registration as a Registered Nurse, demonstrate evidence-based practice, and participate in ongoing professional development. A comprehensive induction, clinical supervision, and leadership support will be provided, along with a peer group of experienced Senior Community Nurses.
