Job summary

CNTW NHS Foundation Trust in Sunderland is seeking a Band 6 Community Practitioner to join the Sunderland South Community Treatment Team. You will assess and treat clients with moderate to severe mental health difficulties, working within a multidisciplinary team to deliver NICE-guided care.

The role requires autonomous working, leadership, and a commitment to high-quality, safe treatment, with opportunities for supervision, training, and professional development within CNTW.

Qualifications

• Registered professional with current practice registration in nursing or allied health.
• Post-registration training in clinical/psychological approaches relevant to the post.
• Experience working in a community mental health setting and CPA/Safeguarding training.
• Knowledge of NICE guidance and community treatment principles.
• Non-Medical Prescribing qualification is desirable.

Responsibilities

• Provide MDT assessment of clients and identify collaborative treatment plans.
• Deliver evidence-based psychological therapies individually and in groups.
• Support the wider team to maintain high standards of care.
• Understand pharmacological approaches to treatment.
• Collaborate with families and other professionals throughout care.
