Tees, Esk And Wear Valleys NHS Foundation Trust is seeking an experienced Mental Health Practitioner to join the MHSOP Redcar and Cleveland team based at Reed Marsh House for a 6‑month fixed term/secondment. You will assess individuals with mental ill health and memory decline, providing interventions and coordinating care across pathways, with duty work on a rota.

An exciting opportunity has arisen for expressions of interest for an experienced Mental Health Practitioner to join our MHSOP Redcar and Cleveland based at Reed Marsh House. This would be for a 6-month fixed term/secondment opportunity. The role will involve assessing people experiencing mental ill health and memory decline from assessment, diagnosis, treatment and transfer back to primary care. The role would involve duty work on a rota basis.
Candidates must have 18 months post-qualifying experience including sign off of appropriate preceptorship or competency framework at the point of application.

As a member of the team, you will provide a service to those individuals who present with mental health issues, with an emphasis on assessing for cognitive impairment. You will be willing to work collaboratively with service users of various needs as well as their carers and families.
You will need to have significant experience of working with people with mental ill health in a care setting, delivering clinical interventions specific to care pathways, in particular the Dementia care Pathway. You will possess excellent communication and interpersonal skills. You are required to have the ability and experience to manage complex cases in partnership with other agencies.
You will encourage ongoing engagement from all staff utilising effective leadership skills. You are required to use your own initiative and make decisions independently for this post.
