The Life Rooms is one of Mersey Care NHS Foundation Trusts innovative services. Our Social Model of Health is designed to support the prevention and population health agendas through promoting health activation and community access.
The Community Inclusion Team aims to enhance the inclusion and wellbeing of under-represented communities within Liverpool. The Team's scope involves collaborating stakeholders, promoting diversity and equality, providing support services, and implementing strategies to create a more inclusive environment for all members of the community. The Team works towards fostering a sense of belonging for individuals of different backgrounds, abilities and experiences.
As a senior Community Inclusion Worker your role will be to provide line management to the Community Inclusion Workers and develop partnerships with those working with multi-ethnic communities.
You will be joining an enthusiastic, innovative and motivated team dedicated to providing high quality services through a social model of health.
The position is a permanent position and involves travelling and working from different community venues in the Liverpool area.

We have an exciting opportunity for a passionate and dedicated individual to join the Life Rooms as a Senior Community Inclusion Worker in the Community Inclusion Team.
The Senior Community Inclusion Worker role is responsible for:

Mersey Care is one of the largest trusts providing physical health and mental health services in the North West, serving more than 1.4 million people across our region and are also commissioned for services that cover the North West, North Wales and the Midlands.
We offer specialist inpatient and community services that support physical and mental health and specialist inpatient mental health, learning disability, addiction and brain injury services. Mersey Care is one of only three trusts in the UK that offer high secure mental health facilities.

We support our staff to do the best job they can and work alongside service users, their families, and carers to design and develop future services together. We’re currently delivering a programme of organisational and service transformation to significantly improve the quality of the services we provide and safely reduce cost as we do so.

1. Develop partnerships with those working with the BAME, asylum seeker, refugee and other socially excluded communities to increase their access to health services and to support them to find, or sustain, meaningful vocational &/or social inclusion activity. 2. Line manage the Community Inclusion Workers; including all aspects of Human Resource Management on a day to day basis. 3. Carry a Community Inclusion Worker workload; supporting members of excluded communities to access health and other services which improve their life chances and decrease their isolation through the development of community access opportunities. 4. Support the Social Work and Research Lead to articulate and develop The Life Rooms Social Model of Health in relation to the Community Pillar and how that works to reduce social isolation and exclusion not only in relation to excluded communities including BAME groups, refugees asylum seekers but also the general population accessing The Life Rooms. 5. Ensure that all Community Inclusion Workers accurately capture and collate all activity data relating to both the service KPI’s and activity pertinent to their role in relation to The Life Rooms. 6. Report any safeguarding issues ensuring concerns are acted upon appropriately and in a timely manner, including the completion of both internal and external records e.g. Pre-EHAT, DATIX etc. Support the Community Inclusion Workers in relation to any more complicated safeguarding or DATIX issues etc, seeking advice as necessary from the Team Leader. 7. As a member of The Life Rooms contribute towards the development of the Social Model of Health and its evaluation
