Erewash Community GP in Derbyshire is seeking an additional Community GP for 4 sessions per week (Mon/Tue/Wed/Fri) starting October 2026. The post provides senior clinical leadership, direct patient care and multidisciplinary governance across community settings in Erewash.

The role supports TeamUp Community Service, with duties including leadership within the TeamUp MDT, managing complex frailty cases, and education and supervision of the team; daily schedule runs from 08:00 to 18:30 with
