Connected Health Group Limited in Magherafelt is seeking a Community Care Manager to oversee care services in the community. You will lead a care team, coordinate schedules, and drive continuous improvement to deliver compassionate, high-quality home care.

This role requires 3+ years in domiciliary care, a full UK driving licence, and proven leadership. You will manage rosters, supervise staff, and work with health partners to ensure excellent client outcomes and safety.

Qualifications

• 3+ years' domiciliary care experience.
• Full, valid UK driving licence and insurance for business use.
• Experience leading a team.
• Experience creating rosters.
• Ability to use initiative to improve service user lives.
• Creative problem solving in community settings.
• Flexible to cover sickness and travel.
• Positive and engaging.
• Excellent communication skills.
• Strong planning and organisation of workload.

Responsibilities

• Maintain relationships with regional coordinators for development and growth.
• Contribute to business growth targets and KPIs.
• Oversee on-call cover in the area (Mon-Sun).
• Direct line management of Care Team including workload, leave, sickness, supervision, appraisals and disciplinaries.
• Liaise with Finance and HR per internal processes.
• Ensure regular team and individual communication.
• Manage 12-week induction and ongoing supervision of new starters.
• Attend client reviews and liaise with Care Managers and Health Trusts as needed.
• Oversee onboarding of new clients and emergency cover when required.
• Report safeguarding and client issues to Line Manager.
• Coordinate with Quality and Compliance re: NISCC and incident management.
• Maintain high-quality client care records and monthly updates.
• Support staff development and handle staff issues confidentially.
