Job summary

Connected Health Group Limited is seeking a Community Care Manager to lead domiciliary care across South Belfast. You will nurture care teams, drive service quality, and maintain client relationships while meeting growth targets.

The role requires on-call responsibilities, staff supervision, and collaboration with HR and Finance teams. The ideal candidate has 3+ years in domiciliary care, holds a UK driving licence, and can lead weekly workloads and client reviews in a community setting.

Qualifications

• 3+ years' experience in domiciliary care.
• Full, valid UK driving licence and appropriate insurance.
• Experience leading a team and creating rotas.
• Strong communication and organisational skills.

Responsibilities

• Maintain relationships with the coordinator to drive growth in the area.
• Lead the care team and manage workloads, leave, sickness, appraisals and disciplinaries.
• Oversee on-call telephone services 7 days a week in your area.
• Attend client reviews and liaise with Health Trusts as required.
• Ensure onboarding of new clients and provide safeguarding reports when needed.
