Qualifications

• A valid driving licence and car available for work.
• Experience in care or rehabilitation settings and able to work in the community.

Responsibilities

• Deliver 1:1 and group rehabilitation sessions to adults with disabilities or long-term conditions.
• Provide advice to families and carers in community settings and home visits.
• Work under supervision of registered therapy staff (physiotherapists and occupational therapists) and contribute to service monitoring and development.
• Utilise EPIC electronic health records and paper-based documentation as part of routine patient care.
