Job description

Senior Clinical Pharmacist (Advanced Practitioner)

Support the PCN and Practices to achieve maximum performance through the Investment and Impact Fund (or equivalent) and QOF, including ensuring good working relationships with Community Pharmacy and supporting Practices to meet prescribing targets.

Provide high-quality patient-facing services, including cover for pharmacy team members during annual leave, study leave, or planned sick leave:

  1. Long-term condition reviews: Take responsibility for care management of patients with chronic diseases requiring medicine optimisation (e.g., COPD, asthma).
  2. Patient-facing clinical medication reviews: Undertake reviews with patients, providing recommendations for prescribing and monitoring to the clinical team. Support patients to optimise medication use, reduce waste, and promote self-care through structured medical reviews.
  3. Specialist prescribing expertise: Develop areas of specialist prescribing aligned with PCN priorities.

Ensure systems are in place for effective care home medication reviews and support the Pharmacy Team in conducting clinical medication reviews with patients. Collaborate with care home staff to improve the safety of medicines ordering and administration.

Provide specialist expertise in medicine use, addressing public health and social care needs to proactively reduce health inequalities in the Connexus PCN area. Play a key role in shared care protocols, clinical research involving medicines, and liaising with specialist and community pharmacists.
