We are seeking a passionate and forward-thinkingSeniorClinical Pharmacistto lead, manage and support our multidisciplinaryteam, delivering high-quality patient care to achieve the requirements of the PCN Clinical Pharmacy team workplan.

Main duties of the job

Working as a clinical pharmacist within the team, the postholder will also be responsible for developing andmaintaining clinically and cost-effective prescribing provision across thenetwork population - improving patient outcomes and helping to relieve serviceand financial pressures across the PCN.

The role provides an exciting opportunity to developleadership and management knowledge and skills in addition to patient-facing,person-centred and integrated care. The post holder will be supported by thePCN Clinical Lead to support integration of pharmacy work into practices.

About us

East Cleveland PCN consists of 5 member practices serving apatient population of approximately 42,000. The successful applicant will beexpected to work in any of these practices to support with service delivery.

Job responsibilities

Clinically, the post holder will be responsible for the care management of patients with chronic diseases undertaking clinical medication reviews to support the proactive management of complex polypharmacy patients. This will include, but not be limited to, elderly people in care homes, patients with multiple co-morbidities and those with learning disability and autism.

As an independent prescriber, the postholder will provide clinical leadership in the delivery of person-centred medicines optimisation, helping to support standardised approaches to care across the PCN and forging strong relationships with both specialist and community pharmacists to promote system-wide medicines optimisation and quality improvement.

The Senior Clinical Pharmacist will coordinate the workload and allocation of workload of PCN Pharmacists and Pharmacy Technicians across the network. The post-holder will provide day-to-day support to team and practices in respect to management of medication and prescription queries. They will support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safe prescribing providing expertise in clinical medicines advice, whilst proactively and reactively addressing both public and social care needs of the network population.

The post-holder will directly provide the duties outlined in the Job Description in addition to any other tasks deemed relevant to the role and providing support and supervision to both Pharmacists and Pharmacy Technicians in the delegation of relevant responsibilities as appropriate.

  • Audit and quality improvement, including the review and analysis of data; risk/issue management and addressing quality/performance in practice

Qualifications
  • Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council
  • Qualified from CPPE Clinical Pharmacist Training pathway
  • Hold an independent prescribing qualification
  • Knowledge and awareness of legal, ethical, professional and organisational policies/procedures relevant to the role
  • Masters or ACP qualification
  • Leadership and management qualification
  • Knowledge and understanding of Primary Care Network arrangements and their contribution to the NHS 10-Year Plan
  • Knowledge, awareness and experience of brief intervention and referral pathways that provide opportunistic advice and support for preventable illness and/or long-term conditions receptive to lifestyle/pharmacological support
