Job description

We have an exciting opportunity for an enthusiastic and proactive pharmacist to join our supportive team delivering innovative pharmacy services to the community.

Role Overview

You will be working as part of a multidisciplinary team supporting our four primary care home teams, delivering patient-centered care. Additionally, you will have the opportunity to provide clinical input into our two community hospital inpatient wards (Beccles Hospital and Carlton Court) and the specialist palliative care unit delivered by St Elizabeth's Hospice.

Key Responsibilities

1. Ensure compliance with medicines legislation, national and local guidance, and manage risks associated with medicines use.
2. Conduct audits, lead improvement projects, and develop action plans in line with the Pharmacy yearly plan.
3. Maintain appropriate governance regarding medicines, including responding to drug safety alerts, clinical guidance changes, incidents, and escalation as needed.
4. Provide professional expertise, advice, support, and training on medicines optimisation policies to health professionals.
5. Organise and participate in training and feedback sessions for clinicians and support staff.
6. Mentor Pharmacy Technicians and support staff, responding to queries supportively and promptly.
7. Deputise for the Operational Lead or Head of Pharmacy when appropriate and agreed.
8. Engage in patient contact, problem-solving, and shaping pharmacy services within a multidisciplinary team.
9. Work autonomously within your sphere of competence, demonstrating efficiency, enthusiasm, and confidence.
10. Communicate effectively with social care, clinical, community teams, patients, and carers.
11. Contribute to improving patient outcomes and ensuring medicines governance compliance with CQC standards.
12. Support learning and development, including undertaking independent non-medical prescribing if not already qualified.
13. Be flexible and willing to travel as the role involves hybrid working.

Candidate Profile
