Senior Clinical Fellow in Obstetrics and Gynaecology

Senior Clinical Fellow (ST3+ equivalent) in Obstetrics and Gynaecology with interest in Acute Gynaecology / Recurrent miscarriage

Applicantsare invited to apply for a post as Senior Clinical Fellow (ST3+ equivalent) inObstetrics and Gynaecology with interest in Acute Gynaecology / Recurrentmiscarriage in Leeds Teaching Hospitals NHS Trust. This is an exciting opportunity to work in afriendly, dynamic, large, teaching acute hospital trust.

Thepost holder will work at a level equivalent to that of a ST3+ trainee.Applicants must have completed Foundation competencies either as part of the UKFoundation programme or demonstrate equivalent training. Applicants shouldideally have experience at working at a ST3+ level for a minimum of 6 monthsideally within the UK. Basic LifeSupport training is essential.

Main duties of the job

Theabove post is suitable for doctors wishing to gain clinical experience at aST3+ level in Obstetrics and Gynaecology.The post offers the successful candidate a wide range of experience inall areas of the specialty. Whilst this post is not recognised for training,the appointee will be supported by a Clinical Supervisor to meet their careerand educational goals.

The post holder will also be encouraged to take partin departmental teaching and quality improvement work.

About us

Thegynaecology service is predominantly based at St James University Hospital(Chancellor Wing) and includes general, urogynaecological, recurrentmiscarriage, endometrial ablation, fertility control, complex contraception,menopause (based at a nearby GP practice), perineal and ambulatory gynaecologyclinics such as hysteroscopy (including outpatient resection), vulval andcolposcopy. Gynaecology theatres and thegynaecology acute treatment unit which includes an ultrasound scan service andoutpatient acute gynaecological procedures, are also located here. Someelective gynaecology work including clinics, day case and overnight theatre andhysteroscopy also takes place at Wharfdale hospital.

Thefertility service is based at Seacroft Hospital.

Thematernity service is based at St James University Hospital (approximately 4,500deliveries per year) and Leeds General Infirmary (approximately 5,200deliveries per year).

Job responsibilities

Thepost holder will work on the second on call (ST3+) rota, and as such will takeon the responsibilities associated with this role. The post holder will besupervised by a Consultant during clinical duties whether elective or on calland will be expected to work as part of a multidisciplinary team, demonstratinggood communication skills with all members of the team.

Theweekly timetable will vary from week to week. It will include sessions inantenatal clinic, delivery suite, gynaecology outpatients, gynaecology theatreand the emergency gynaecology assessment unit. These sessions are aimed todiversify the post holders clinical experience, to provide a service and toaccommodate the rolling on call rota of long days and night shifts on labourward and the gynaecology acute treatment unit. On call frequency is 1 in 9.

Antenatalclinic The post holder will be expected to:

- Conduct the initial antenatalassessment of women, identify risk factors in the booking history, and consultor refer appropriately under the supervision of a senior registrar orconsultant.

- Counsel women adequately about theinvestigations offered during pregnancy, interpret the results, communicate thesignificance of abnormal results to the patient and arrange further managementif needed.

- Detect and manage commonly occurringdisorders in pregnancy, recognising their limitations, and referring to othercolleagues in the team when necessary

- Recognise the roles of themultidisciplinary team

- Demonstrate good communication withpatients and staffLabourWard The post holder will work under the supervision of a ConsultantObstetrician on call or on hotweek and:

- Work in conjunction with the seniorMidwife

- Be aware of the wishes and concernsof women in labour, and maintain their dignity, autonomy, physical andpsychological comfort

- Help in monitoring of fetal conditionin labour

- Assess and manage abnormal progressin labour

- Have a chance to perform instrumentaland operative deliveries.

- Assist in the management of thirdstage complications

- Assist in the management of maternalcollapse and neonatal resuscitation

GynaecologyOutpatients The post holder will be allocated to attend specific Gynaecologyclinics, but are also encouraged to attend a variety of the more specialisedclinics which provide valuable learning opportunities when possible. The postholder will:

- be able to take a gynaecologicalhistory and perform an appropriate examination with respect for the womansdignity and autonomy

- initiate investigation and managementof common gynaecological problems such as disorders of menstruation,infertility, incontinence, menopausal symptoms, psychosexual problems andgenital tract infections.

- understand cervical screening, themanagement of pre invasive and invasive disease and treatment follow up

- Demonstrate good communication withpatients and staff

GynaecologyAcute Treatment Unit The post holder will work under the supervision of theConsultant Gynaecologist on call or on hotweek and:

- Take referrals from generalpractitioners, emergency department and other specialities concerning womenwith early pregnancy or acute gynaecological conditions

- Assess and initiate management and /or treat common early pregnancy and acute gynaecological conditions

- Participate in acute gynaecologyprocedures including surgical management of miscarriage, manual vacuumaspiration in office setting, Word catheter insertion for Bartholins andlaparoscopic surgery.

- Reviewwomen after elective gynaecology surgeryGynaecologyTheatre The post-holder will observe, assist or undertake procedures intheatre with an appropriate level of supervision.

Thepost-holder will always be resident in the hospital when on duty. TheConsultant Obstetrician will be resident between 08.30-22.00 on weekdays and08.30 and 17.00 on weekends. The Consultant Gynaecologist will be residentbetween 08.30 and 17.00 on weekdays and will conduct a resident ward round onweekends. They will remain resident beyond these times if required but willotherwise be on call from home.

Thepost complies with the New Deal requirements and the European Working TimeDirective.
