High Intensity User FellowshipWe are looking for an EM clinician with a passion for service development and quality improvement to join our team in managing High Intensity Users (HIU) in our Trust. The post holder will assist the clinical lead for HIU in setting up a Multi-Disciplinary Team (MDT) service, engaging with the North West London Integrated Care System (NWL ICS) and our partners in Mental Health, LAS, General Practice and Social Work. The service runs weekly MDTs, along with a social prescriber, to create individual care plans for patients identified as High Intensity Users and provide them with the holistic care they need. The successful applicant will also attend monthly meetings at a Pan-London and NWL level for HIU.
This post will therefore combine several important managerial aspects of working as a senior clinician in the NHS, including chairing meetings and liaising with multiple agencies to create and agree care plans.
