Job summary

The Wellington Hospital, part of HCA Healthcare UK, is seeking an experienced Clinical Case Manager with paediatric and adult care expertise. This on-site role is full-time (37.5 hours per week) with options 8am-4pm or 9am-5pm, based in London.

You will coordinate complex patient journeys, lead discharge planning, and liaise with clinicians, families and third‑party payers to optimise outcomes. Suitable candidates will be a Band 6+ nurse or clinician with inpatient experience across both adult

Qualifications

• NMC registered adult nurse with paediatric exposure.
• Hands-on inpatient experience in adult and paediatric settings.
• Experience in clinical leadership and discharge planning.
• Experience with utilisation reviews and authorisation processes.
• Senior-level experience (Band 6 or above) with leadership and team coordination.
• Ability to lead complex clinical decisions and coordinate with multiple stakeholders.

Responsibilities

• Assess, plan, coordinate and review patient care and discharge planning across adult and paediatric services, starting from admission.
• Act as liaison between patients, families, clinicians and third-party payers for safe, effective and timely care.
• Complete utilisation reviews and obtain authorisation for treatment and ongoing care.
• Contribute to multidisciplinary discussions and coordinated care and discharge decisions.
• Promote patient advocacy, efficient resource use and high-standard documentation.
