Job overview
Evelina London is at the heart of a network of children’s health services in Lambeth and Southwark and across South-East England. We provide a comprehensive range of children’s health services, so that we are able to look after patients from antenatal diagnosis through childhood, into adolescence and on into adult services.
The post holder will work as an autonomous practitioner providing high quality specialist nursing care to children in their homes. The post holder will be responsible for facilitating early discharge, reduce length of stay and prevent hospital admissions. The post holder will provide clinical supervision to other staff, students and may undertake research, lead clinical audits in own specialist area. Ensuring the safety and well being of services users, and always working in a manner that promotes dignity and human rights through the adoption of person centred care principles.
Car driver essential. Pool cars (inc. automatic) are provided for team use.

Main duties of the job
The Evelina Children's Community Nursing Team works closely with acute services in local and specialist hospitals to ensure a seamless pathway of care for the child on discharge from the acute setting. Using evidence based practice and knowledge you will provide continuing responsibility for the assessment, development, implementation and evaluation of holistic nursing care for patients with both acute, long term and complex conditions.
You will provide expert support, training and advice to health visitors, school nurses and other professionals to ensure the broad range of needs of the client caseload are met in all community settings including home, nursery, specialist and mainstream school. The successful candidate will carry out and support specialist procedures in the community eg administration of intravenous antibiotics and chemotherapy, administration of medicines, wound care, care of central venous access devices, changing of tracheostomy and naso-gastric tubes.
Previous experience of working in a community setting and/or working autonomously with acute nursing skills is essential for this post.

Detailed job description and main responsibilities
The job role is community based across Boroughs of Lambeth and Southwark and works collaboratively with Primary, secondary and emergency care providers.
We will provide the successful candidate with the relevant in-house training and further education supported by our education department. You will be enrolled on a community development pathway that will include Evelina London Foundations in Children’s Community Care (EL FiCC) and Evelina London Paediatric Advanced Clinical Assessment (El Paca) that will mirror an enhanced skills pathway.
