Wiltshire Council is seeking a Level 3 Social Worker for the Continuing Healthcare team to assess adults who may qualify for NHS CHC funding. You will work with health and social care partners to determine eligibility, funding arrangements and care planning to support individuals.

The successful candidate will be registered with Social Work England, with up-to-date safeguarding knowledge and experience supporting vulnerable adults.

Qualifications

• Qualified and registered Social Worker with Social Work England.
• Up-to-date safeguarding knowledge.
• Experience supporting vulnerable adults and multi-disciplinary teams.
• Strong communication and professional development mindset.

Responsabilités

• Assess adults with complex health and care needs for CHC eligibility.
• Support CHC eligibility decisions and funding arrangements.
