Job description

Job summary: An exciting opportunity has arisen within our Personalised Care Department in Trafford Locality. We are looking for highly motivated and experienced RMNs, RGNs OR Allied Health Professionals who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support Commissioners to deliver robust support planning for children and young people with complex disabilities and their families who are at risk of admission by carrying out community Care and Education Treatment Reviews (CETRs) and promoting multi-agency working for individuals with the most complex needs. The ability to challenge where appropriate is essential in this role.

Main duties of the job: Enabling preventative and integrated approaches which support children and young people with LD and/or Autism and behaviour that challenges to remain in their local communities by ensuring CETRs are carried out on time and to a high standard and ensuring that admissions to residential or CAMHS inpatient units will be prevented through quality risk management processes.

Establish and maintain effective links and good communication with all stakeholders including Local Authorities, NHS England, specialist services, Care Provider organisations and specialist mental health and learning disability service providers (NHS and independent sector).

Support processes and mechanisms that will ensure that patients in specialist mental health/learning disability services receive high quality and clinically appropriate treatment in accordance with identified individual care pathways and in the least restrictive setting.

Support the commissioning of individual packages of care in the community and monitor assurance outcomes whilst working to ensure individuals are safe and packages of care meet their needs.

Provide clinical case management for existing and new transforming care patients, including but not limited to unscheduled participation in CTRs to facilitate crisis intervention.

Work collaboratively with stakeholders to agree appropriate care and treatment needs and facilitate timely discharge.

Person Specification

Qualifications Essential:

• Educated to diploma level with extensive experience of Mental Health Service Provision including the Mental Health Act 1983.
• Post registration specialist knowledge and gained experience and evidence of CPD.
• Educated to a post graduate degree level or equivalent experience in Healthcare Service.

Desirable:

• Undertake personal and professional training and enhanced professional development in general and in relation to, patient discharge processes and healthcare funding processes.

Experience Essential:

• Significant diverse post qualification experience health related agencies, in children's services, or related private or voluntary organisation, of a magnitude that enables the post holder to work independently
• Experience of chairing meetings with clients, family members and/or carers and other multi-agency professionals; and making decisions and recommendations on difficult and contentious issues where there may be a number of courses of action.

Knowedge Essential:

• Joint working with partner agencies
• High level working knowledge of the relevant legislation and theories underpinning the provision of services to the relevant client group - Mental Health legislation, NHS 10-year plan, Children Act 1989, Children and Families Act 2014, Mental Capacity Act 2009
• Knowledge and understanding of all relevant policies in relation to S117 of the Mental Health Act 1983 and more broadly the Mental Health Act 1983 and all its key functions.
• High level knowledge of the Transforming Care agenda and Building the Right Support
• Expert understanding of relevant practice theories including Positive Behaviour Support
• Comprehensive working knowledge of safeguarding procedures and act in accordance of local safeguarding procedures.
• Knowledge and experience of clinical governance, within their current/past field of practice.
• Comprehensive knowledge of data protection and confidentiality issues.
