Overview

Are you an Independent and supplementary Prescriber or willing to train? Are you a registered Mental Health Nurse? Do you have a passion for improving access to high-quality, innovative, and compassionate mental health services?

As a First Contact Mental Health Practitioner, you will work as part of a dynamic team to support Primary Care Networks (PCNs) in delivering effective mental health support within primary care. This specialist role operates at an advanced level within GP practices.

The role involves triage, assessment, and expertise in networking and navigation to facilitate access to various mental health services, ensuring patients receive the appropriate care at the right time and place.

Main Duties

  • Conduct clinical duties within GP practices as an autonomous Mental Health Practitioner, providing comprehensive triage and risk assessments without prior GP consultation. Appointments are typically 30 minutes via face-to-face, telephone, or home visits.
  • Diagnose within scope and develop management plans based on evidence-based practice and NICE guidance.
  • Provide short-term risk management interventions and develop safety and self-harm management plans with patients and their significant others.
  • Identify cases where risk cannot be managed safely in primary care and make timely referrals to specialist mental health services, maintaining patient safety and liaising closely with these services.
  • Make referrals across primary, secondary, and third-sector mental health, health, and social care services.
