Job summary

Devonport Health Centre is seeking an Advanced Practitioner with nursing or paramedic registration to join our multidisciplinary team in Plymouth. You will be qualified to ANP standard and ideally be a Non Medical Prescriber or willing to undertake the course.

You will work across well-woman/man clinics, chronic disease management, and vaccination programmes, ensuring safe, high-quality patient care in a supportive practice environment.

Qualifications

• Registered Nurse with NMC registration in the UK.
• Masters degree or higher in Advanced Nursing Practice or equivalent.
• Evidence of working at an advanced practice level per primary care frameworks.
• Current NMC revalidation requirements met.
• Desirable Independent Nurse Prescriber qualifications.

Responsibilities

• Develop and embed health promotion and wellbeing programmes.
• Manage patients with acute and chronic conditions, providing expert advice.
• Implement and evaluate individual treatment plans for chronic disease patients.
• Identify and support patients at risk of long-term conditions.
• Provide advanced, specialist nursing care within evidence-based guidelines.
