South West London and St George's Mental Health NHS Trust is seeking an Acute Care Pathway Coordinator to maintain ACP operations within bed capacity during OOH periods and provide leadership during working hours. The post holder will coordinate access to Acute, PICU, Older Adult beds, Crisis Hostels, PDU and HBPOS capacity, prioritising based on clinical need and risk.

They will lead suspension of delays through discharge planning and work with senior clinicians to reconstitute capacity,

Qualifications

• Registered General or Mental Health Nurse.
• Post Qualification - Teaching qualification (ENB 998, Mentorship) or equivalent.
• Demonstrated leadership and management skills in a healthcare setting.
• Experience in risk management and discharge planning.

Responsabilités

• Coordinate bed management across ACP, including weekends and bank holidays.
• Lead ACP bed management meetings and ensure discharge plans with clear timeframes.
• Liaise with senior psychiatry staff, Home Treatment Teams and Integrated Recovery Hubs for appropriate patient pathways.
• Support Assessing and Triage services in identifying admission alternatives.
• Represent the Trust values in interactions with staff and patients.
