Job summary

Leaders In Care Recruitment Ltd is currently recruiting for an experienced Social Worker to join a well-established North Wales 0–25 Disabilities Team on a long-term contract. The role offers a manageable caseload, flexible working patterns and hybrid arrangements to support ongoing, relationship-based practice.

Qualifications

• Qualified Social Worker registered with Social Work England.
• Experience in Children with Disabilities, 0–25 disabilities, safeguarding, child in need, assessment or transition teams is required.
• Strong knowledge of safeguarding legislation and practice relating to children and young people with disabilities.

Responsibilities

• Manage a caseload of children, young people and young adults aged 0–25 with disabilities and complex needs.
• Complete Child and Family Assessments, statutory assessments and reviews; Undertake Care Act, transition and Short Break assessments where appropriate.
• Develop, implement and review person-centred care and support plans.
• Work closely with families, schools, health professionals and multi-agency partners; identify, assess and manage safeguarding concerns in line with statutory guidance.
• Support Child in Need, Child Protection and Looked After Children processes where required.
• Contribute to transition planning for young people moving into adulthood; prepare clear, high-quality assessments, reports and court documentation when necessary.
• Ensure the views, wishes and feelings of children and young people remain central to all planning and decision-making.
