Overview

We have a fantastic opportunity available for a SEND Casework Practitioner to join our friendly SEND team at Wokingham Borough Council, on a permanent, part time basis.

Responsibilities
  • Develop and maintain Education Health and Care Plans from initial application and assessment through to the annual review process.
  • Work with families of children and young people with Special Educational Needs and Disabilities in a person-centred relational approach.
  • Coordinate service provision across education, health and care to deliver holistic plans for individual children and families.
  • Contribute to service improvement and collaborate with education settings, health, and care partners in line with local offer provisions.
  • Manage a caseload of children and young people with SEND; act as a key point of contact and engage with families, schools, colleges and other partners as required.
  • Prioritise statutory processes for vulnerable children and young people with Education, Health and Care Plans to identify appropriate provision without delays.
Candidate requirements
  • Understanding of SEN legislation and procedures, including knowledge of legislation related to SEN (e.g., Children’s Act, Code of Practice, DDA).
  • Experience of working with children and/or families with SEN.
  • Experience of managing complex casework involving mediation or conflict resolution in an educational environment.
  • Ability to write detailed and complex reports independently using information from internal and external colleagues.
  • Good IT skills, including Microsoft Office and database systems.
  • Ability to travel to various locations in the borough to develop relations with educational settings and attend training with key stakeholders.
