Job description

Our community healthcare teams in adults and children's services are employed by Manchester University NHS Foundation Trust and deployed to us, linking closely with our colleagues in Manchester Local Care Organisation. Our teams work as one, and together we will make a significant difference to the health of people in Trafford.

The post holder(s) will work as part of the School Health team to promote and maintain the health and well-being of children, young people, and families. This is an integrated role within the TLCO, providing services across various community settings.

The post holders will be responsible for implementing the Healthy Child Programme 5-19 years, assessing healthcare needs of children, young people, and their families. Responsibilities include:

  • Identifying and addressing health needs
  • Raising awareness of health issues
  • Facilitating health-promoting activities
  • Influencing health policy

They will promote safeguarding children, working within a multi-agency team, and ensure that named staff members have annual appraisals and development plans.

While primarily working within one team, applicants may be required to work at other locations to ensure equitable service delivery. Core service hours are Monday to Friday, 8:00 am to 5:00 pm.
