This role offers the chance to work with a varied caseload of patients receiving acute medical care in their own homes, allowing for a truly personalised approach to treatment.
The successful candidate will be based at the Woodend Hospital site in Aberdeen and will play a key role in ensuring patients receive the highest standard of assessment, care, and treatment. The post holder will work both independently and as part of a multidisciplinary team, collaborating with Consultants, GPs, Advanced Nurse Practitioners, Nurses, Physiotherapists, Pharmacy staff, Occupational Therapists, and Health Care Support Workers. Duties include providing patient-centred care in the home setting and managing a varied caseload of patients receiving acute medical care.
We are seeking motivated team members with a commitment to delivering high-quality patient care. Preference will be given to applicants with current hospital or community experience. Applicants must meet the requirements of the person specification to be considered for these posts.
