1. Manage established patient demographics with low administrative duties and home visits.
2. Work with support from an experienced multi-disciplinary team including Practice Nurses, ANPs, Pharmacists, and HCAs.
3. Offer 6-8 sessions per week, with flexible working days and session timings.
4. Manage a list size of approximately 7,000 patients.
