Lawford Surgery is a semi-rural dispensing practice serving around 5,500 patients. We are looking for a fully qualified GP with GMC registration to join our friendly team in primary care.

Essential duties include managing chronic diseases, developing care plans with patients, delivering health education, and completing patient notes to agreed standards. Applicants must be on the NHSE medical performers list and compliant with appraisal and revalidation processes.

Qualifications

• Essential: Good understanding and experience of chronic disease management and primary prevention and screening services.
• Be on the NHSE medical performers list and aware of practice policies and guidelines.
• Annual appraisal and revalidation (when appropriate).

Responsabilités

• All general duties and responsibilities associated with a GP working within primary care.
• Developing care plans for health in consultation with patients and in line with current practice disease management protocols.
• Providing health education and recording clear consultation notes to agreed standards.
• Collecting data for audit purposes and complying with QOF requirements.
• Delivering enhanced services and compiling/issuing computer-generated acute and repeat prescriptions.
