Village Health Group (VHG) is seeking a Salaried GP to join our four-site practice in Rushcliffe. The role offers flexible sessions, a mix of face-to-face and digital consultations, and opportunities for GP registrar supervision within a busy training environment.

About Village Health Group

Village Health Group (VHG) is a large GMS practicecaring for around 28,000 patients across Rushcliffe, SouthNottinghamshire. The practice was formed in April 2020 following the merger ofEast Leake Medical Group and Keyworth Medical Practice, both previously rated Outstandingby the CQC.

We operate across four sites, with our mainsurgeries in East Leake and Keyworth and branch surgeries in Ruddington andSutton Bonington. We are also a part-dispensing practice.

Our clinical team includes 10 GP Partners,salaried GPs, Advanced Clinical Practitioners, Practice Nurses, HealthcareAssistants, Clinical Pharmacists, Physician Associates, Social Prescribing LinkWorkers and Care Coordinators. We work closely with community nursing andtherapy teams and have a strong multidisciplinary approach to patient care.

We use SystmOne and are supported by anexcellent administrative team and an active Patient Participation Group.

We are an accredited training practice for GPRegistrars and University of Nottingham medical students, an active researchpractice, and a leading member of both PartnersHealth and RushcliffePrimary Care Network.

Job Purpose

Provide general medical services to the patients of theVillage Health Group.

Provide clinical assessment and deliver treatments topatients who have undifferentiated health care requirements in order to promotegood health and prevent unnecessary acute hospital attendances.

Utilise advanced clinical skills and competencies inprescribing, health assessment, medicines management, leadership and casemanagement

Maximise patients health and reduce risks that contribute toill health.

Act as an autonomous practitioner who is open to theflexibility required for the role.

Provide clinical support and advice to staff to promotecaseload management techniques to reduce unplanned hospital admissions causedby poor control of long-term condition

Work collaboratively across primary, secondary, social careand the voluntary sector to establish a whole systems approach to managinghealth care needs.

Key Responsibilities

Work with the Primary Healthcare Team on a day to day basisfacilitating effective practice, efficient use of resources and smooth servicedelivery.

Clinical

Using expert knowledge, clinical skills and autonomousdecision making skills, work collaboratively with other members of the primaryhealth care team to provide a high standard of care.

Comprehensively assess, review and evaluate the needs ofpatients with undifferentiated health care needs to improve their physical,psychological and mental health well being to promote self care and reduce theneed for hospitalisation

Accurately manage patients health within the practice,undertaking clinical assessment and providing the necessary treatment includingprescribing using local and national guidelines as appropriate

To develop specialist clinical skills (in self and others)to undertake interventions consistent with evidence based practice in line withnational and local policy.

Develop plans of care in conjunction with patients through:application of clinical knowledge and, analysis of symptoms and data,identification of risk factors associated with those conditions, recognition ofearly signs of acute illness, involving the patients and the carers in the careplan and ensuring it is understood, documenting safety netting plans andprocesses

Use knowledge of medicines and prescribing skills toinfluence the reduction of risk and complications associated with medicationand polypharmacy.

To utilise effective communication to aid onward referralsto specialists as appropriate. Facilitating a coordinated primary health careresponse to patient needs

Ensuring effective communication channels and sharing ofinformation that is beneficial to the patients care and avoids conflict oftreatment prescribed by other professionals

To work in partnership with patients, carers, GPs, and otherhealth professionals as appropriate to promote, develop, initiate and implementproactive care.

Link with existing services to facilitate care pathways andpreventing hospital admissions where possible

Act as a role model so that patients receive the mosteffective care possible through encouraging optimum management of healthcareconditions and acting in the patients interests at all times

Encourage and empower patients and their carers in casemanagement by providing accurate up-to-date information about diseaseprevention, progression and outcomes to embed a culture of self-management

Highlight and report any Safeguarding concerns that mayarise, working closely with the Safeguarding Lead where required

Records accurate and contemporaneous I.T. based consultationnotes to agreed standards using System One.

Contribute to the collection of appropriate data for QOF andother primary care DES - LES and other services
