We have a permanent team of two clinical partner GPs, four salaried GPs, an established nursing team, PCN Pharmacy team, First Contact Physiotherapist. We pride ourselves on delivering patient-focused medicine with continuity of care at the heart of what we do.
Provide high quality, patient centred primary care by assessing, diagnosing, and managing a broad range of acute and chronic conditions. Deliver consultations via face to face, telephone, and digital platforms, and develop appropriate management plans. Prescribe medications, request and interpret investigations, and make referrals to secondary care where required. Promote health and wellbeing through preventative care and patient education. Maintain accurate clinical records and work collaboratively within a multidisciplinary team to ensure safe, effective, and efficient patient care.
