Thepost holder will support the GP Partners in delivering the full range ofgeneral medical services as part of the multi-disciplinary team looking afterour registered population, ensuring the highest standards of care for ourpatients.
StBede Medical Centre is located in North Sunderland and serves a currentpopulation of 9,300 patients.

Wehave 5 GP Partners 3 full time male and 2 part-time female.
Wehave a full complement of multi-disciplinary team members including 2 PracticeNurses, 2 Student Nursing Associates, 1 HCA, 1 Mental Health Practitioner, 1Frailty Nurse and other allied health professionals as well as full complementof administration team.
Clinical Responsibilities
In general, the post holder will be expected to undertake all thenormal duties and responsibilities associated with a General Practitionerworking within primary care including:
The post holder will undertake a variety of duties includingface to face, telephone and electronic consultations, visiting patients homes,checking and signing repeat prescriptions, dealing with queries, paperwork andcorrespondence in a timely manner.
Making professional, autonomous decisions in relation topresenting problems, whether self-referred or referred from other healthcareworkers within the Practice.
Assessing the health care needs of patients withundifferentiated and undiagnosed problems.
In consultation with patients and in line with current practicedisease management protocols, develop care plans for health management.
Making appropriate and timely referral of patients for furtheropinion and treatment following relevant guidelines.
Provide counselling and health education as necessary.
Admit or discharge patients to and from caseload and refer themto other care providers internally and externally as appropriate and in linewith practice protocols.
Comply with all relevant clinical governance and Practicepolices and protocols.
Complete clinically related administrative and non-clinicalduties needed for the delivery of the service.
Record clear and contemporaneous consultation notes to agreedstandards.
Demonstrate commitment to lifelong learning, and audit andeffectiveness to ensure evidence based best practice.
Compile and issue computer-generated acute and repeatprescriptions
Meet professional learning and CPD requirements.
Develop effective relationships with colleagues throughout thepractice.
Participate in targeted health promotion activity to benefitpatients.
Timely assessment of incoming electronic and papercorrespondence relating to both patient and nonpatient information as required.Using the appropriate electronic systems (EMIS)
To complete insurance, DWP, DVLA and other reports relating topatients under your care.
To complete safeguarding requests for information as allocated.

Practice Responsibilities
Contribute to theevaluation/audit and clinical standard setting with colleagues.
Ensure clinical database is updated promptly and accurately asrequired for practice protocols, QOF, Enhanced Services etc.
Participate in the auditing of practice activity as required.
Provide reports to both NHS and non-NHS agencies as required.
Responsible for health and safety of themselves and others andfor reporting and actively co-operating in Practice Health and Safety Policiesand procedures.
Process and action incoming patient and hospital correspondence.
Attend in-house meetings, including clinical, palliative care,safeguarding, significant event audit and other meetings as deemed necessary.
Ensure the practice complaints system is adhered to at alltimes.
Awareness and compliance with all relevant guidelines e.g. dataprotection, confidentiality and health and safety.
Maintain current CPR and anaphylaxis certification andchild/adult safeguarding training in line with local policies.
