Job description
Overview

An exciting opportunity to join our multi-disciplinary PCN team which consists of 6 GP Practices, Clinical Pharmacists, Paramedics, Social Prescribers, Physiotherapists, Care Coordinators, Health Coach and Pharmacy Technicians. Prestwich PCN consists of 6 practices that work collaboratively and have done for many years. All practices within the PCN are CQC rated good or above and are supported by experienced nursing staff, ARRS staff, practice managers and administrative teams.

This post is funded through the Network Contract DES. You must be within 2 years of GP qualification and not have been substantively employed as a GP previously. The post will require you to work across no more than 2 of the practices, offering a great opportunity to gain experience across different practices.

We require 4 sessions on a Wed and Fri across The Birches and Greylands practices and 2 sessions on a Fri at Longfield practice. We are very flexible on the number of sessions per week — please indicate the number of sessions as a preference in your application.

Main duties

You will manage a caseload and work autonomously on a day-to-day basis. To support your development, you will have a dedicated GP assigned to carry out professional supervision. You will undertake several duties governed by the GMS contract including:

  • Face to face and telephone consultations
  • Patient home visits and care home visits
  • Checking and signing repeat prescriptions
  • Administrative tasks namely dealing with queries, paperwork and correspondence as required
  • Cover all clinical management of in-surgery patients as appropriate including initiating investigations, reviewing results and making referrals to secondary care or to other providers as appropriate
  • Develop care and treatment plans in consultation with patients
  • The post holder will need to have full access to a car to enable full geographical working across Prestwich PCN and work flexibly to meet the needs of the PCN.

Job responsibilities

Main duties of the job

  • Autonomous general practitioner working independently and with other health care professionals to assess, diagnose and treat patients in primary care, including home visits.
  • Provide expert professional advice to patients, carers and colleagues; maintain clinical excellence and be professionally accountable in line with GMC code of conduct.
  • Full access to a car to enable geographical working across Prestwich PCN and flexible work to meet PCN needs.

  • In accordance with the practice rota, undertake a variety of duties including surgery consultations, telephone consultations, home visits, signing repeat prescriptions, dealing with queries and correspondence in a timely fashion.
  • Make professional, autonomous decisions related to presenting problems; manage undifferentiated problems; develop care plans; prescribe within formulary guidelines; refer to other providers as appropriate.
  • Participate in extended hours enhanced service and fair share of home visiting sessions.
  • Experience with audit and be able to lead audit programmes.
