About the Practice

Based in coastal Plymouth

Multi-disciplinary team including ACPs, ANPs, Paramedics, Pharmacists, Social Prescribers, PNs, and HCA support

Up to 12 sessions per week -negotiable and flexible

15-minute appointments with balanced face-to-face and telephone mix

Capped clinical contacts and dedicated clinical admin time

Regular clinical governance meetings and informal catch-ups
