A new 18-month salaried frailty GP post at Launceston Medical Centre, funded by the Launceston and Tamar Valley Integrated Neighbourhood Team. The work is mostly proactive clinical care for people over 65 with moderate to severe frailty, including medication review and home visits, alongside co-designing a neighbourhood frailty model and aiming to cut acute admissions. The post holder also acts as Frailty Lead GP, supervising practice colleagues and advising across the neighbourhood. Applicants need GMC registration and experience of managing complex older patients.
What the job involves
- Review patient records identified through population-health or risk-stratification processes to assess suitability for proactive frailty care, and keep accurate clinical records and referrals arising from own work
- Carry out proactive face-to-face or telephone reviews, including medication review, falls assessment, long-term condition review and multimorbidity assessment
- Provide medical review for selected same-day or complex frailty presentations where the usual GP is not available
- Undertake and support structured medication review and appropriate deprescribing, balancing safety, treatment burden and patient priorities
- Provide medical supervision and advice to practice staff on the Frailty Test and Learn, including the social prescriber, frailty nurse and care coordinator
- Provide medical advice to the wider INT team, supporting shared clinical decision-making and escalation
- Oversee implementation of the Frailty Test and Learn within the practice, working with social prescribing, the Health Hub and managers, and act as Frailty Lead GP
- Work with the INT to improve frailty care coordination, reduce duplication and remove barriers to collaboration across organisations
- Collaborate with system partners on the pull model to support earlier and safer discharge from acute care
- Contribute medical expertise to holistic assessment, personalised care planning and comprehensive geriatric assessment across the multidisciplinary team, and undertake home visits where GP assessment is necessary
- Contribute to advance care planning, treatment escalation planning and end-of-life discussions, and respond to safeguarding, capacity, carer strain and self-neglect concerns
