Job description

We are excited to recruit to the post of Safeguarding Care Coordinatorwithin the East Primary Care Network (PCN) to work alongside the SeniorSafeguarding Care Coordinator and support outstanding safeguarding governanceand working relationships with partner organisations across the PCN.

The post holder will be primarily workingfrom home. There will be occasionswhen working from GP practice and community settings is needed.GP Practices based working in the East Herefordshire PCN include LedburyHealth Partnership, Nunwell Surgery, Colwall Surgery and Cradley Surgery.Mileage ispaid at 45p per mile for additional travel that exceeds the distance fromhome address to the base practice.

Main duties of the job

The Safeguarding Care Coordinator postprovides a rewarding opportunity to work alongside Lead GPs and Practiceteams to develop efficient processes and systems at the very heart of generalpractice. They will support member practices to;

Providecentral co-ordination for safeguarding information and communication to helpGPs plan care for vulnerable patients

Maintainconsistent protocols for safeguarding administration, document handling,record management and communication with partner organisations

Support multi-disciplinary team working -communicating between teams who have a joint role in looking after vulnerablepatients; the Primary Care Network team, Multi Agency Safeguarding Hub and the ICB.

Job responsibilities

JOB PURPOSE:

The Adult and Child Safeguarding Care Co-ordinator in the East PCN will have a key role in supporting the Senior Safeguarding Care Coordinator and Safeguarding Lead GPs to protect our most vulnerable patients.

The role will:

  • support the Senior Safeguarding Care Coordinator to work within East PCN GP Practices and provide central co-ordination for safeguarding information and communication to support care planning.
  • be GP-facing with the core responsibility of maintaining consistent protocols regarding safeguarding information across the network.
  • provide information and support to colleagues, while overseen by the Senior Safeguarding Care Coordinator.
  • support the audit and research of safeguarding practice within the PCN and disseminate learning to colleagues to improve safeguarding practice across the PCN.
  • be responsible for day-to-day safeguarding administration, document handling, record management and recording communications with partner organisations.
  • support multi-disciplinary team working and provide assistance with liaison between the Primary Care Network team, Multi Agency Safeguarding Hub and the ICB.

This post will be supervised and line-managed by the Senior Safeguarding Care Coordinator for the East PCN. Clinical supervision (as appropriate) will be provided by designated practice Safeguarding Leads. Taurus Healthcare will provide support with employment and safeguarding liaison.

Qualifications
  • GCSE English or equivalent level.
  • GCSE Mathematics or equivalent level.

Experience
  • Working knowledge of Microsoft office Word, Excel, Outlook and PowerPoint and using video calling software e.g. Microsoft teams.
  • Awareness of safeguarding and the associated legislative framework

  • Experience of working in a health or social care setting
