Job summary

The purpose of RRRT is to work with patients and service users to maximise their physical and social functioning to enable them to remain within the community for as long as possible. The MDT undertakes all elements of hospital discharge work including hospital liaison and case finding. The aim of the service is to reduce inappropriate hospital admission, minimise hospital length of stay and support vulnerable clients at home, this role works within a multi-disciplinary, multi-agency team and in partnership with primary health care and social care professionals, providing expert case management, clinical assessment, diagnosis and individualised treatment plans, promoting self-management.The Therapy Assistant will work under the supervision of professionals in the MDT to support patients/service users to regain their independence.

Main duties of the job

To provide support to qualified therapists and nurses via facilitation of rehabilitation programmes with clients whose function and mobility are below their usual level, encouraging, enhancing and progressing their rehabilitation to the best of their abilities. To promote and facilitate rapid responses for clients who require delivery and fitting of small aids and equipment in their homes in order to facilitate their continued safe and independent function and mobility.

Carry out client risk assessment as agreed with the occupational therapists and physiotherapists and provide feedback on clients physical and occupational performance and skills deficits.Carry out client treatment and ongoing rehabilitation as agreed with the occupational therapists and physiotherapists and as per service protocol and feedback on clients physical and occupational performance.Provide client instruction, direction and guidance during assessment and intervention and adjust own clinical interventions based on clients needs during the intervention. To evaluate effectiveness of the intervention in discussion with supervising therapist, and modify programme accordingly.Provide above client interventions in the patients home or in a community setting as appropriate.Use communication, reasoning and negotiation skills to establish a therapeutic relationship with clients and identify barriers and possible solutions to communication difficulties.Feedback effectively to multi-disciplinary team and external agencies on clients performance and progress towards identified rehabilitation goals.Comply with all relevant national and local policies and procedures.Respect the individuality, values, cultural and religious diversity of clients and contribute to provision of a service sensitive to these needs.

Exercise good personal time management, punctuality and consistent reliable attendance.
