RRIC Community Practitioner - Occupational Therapist Buckinghamshire Healthcare NHS Trust

Join our team as a RRIC Community Practitioner - Occupational Therapist at Buckinghamshire Healthcare NHS Trust.

About the Role

We are expanding our innovative Rapid Response & Intermediate Care (RRIC) service across Buckinghamshire. You will work with our Aylesbury team across Intermediate Care and Community Physiotherapy.

Responsibilities

• Provide expert interventions and rehabilitation as part of a dynamic team responding quickly to crises.
• Help patients stay in their own homes, supporting recovery, early hospital discharge, and end-of-life care.
• Contribute to triage, assessment, care planning, and case management of patients and carers.
• Collaborate with multidisciplinary team members including Physiotherapists, Nurses, Paramedics, Pharmacists, and Healthcare Assistants.
• Support development of clinical pathways and work across Intermediate Care and Physiotherapy pathways.
