Do you have a friendly 'can do' approach to life, and experience working as part of a health or social care team? We are looking for you to work as part of the Aylesbury Rapid Response & Intermediate Care team supporting patients in their own homes, providing personal, social, and rehabilitative care. The overall aims of the Rapid Response & Intermediate Care Team are to avoid inappropriate admission of patients into hospital and facilitate timely discharge into the community. The team is committed to ongoing education on the job, and development opportunities are available within the organisation. Successful applicants who don't already have a Level 3 Qualification in Health and Social Care will be expected to work towards that in post.
