Team Composition

The team is a multidisciplinary group comprising Consultant Psychiatry, Non-Medical Prescribers, Assertive Outreach Workers, Recovery Coordinators, Neuropsychologists, and a Mental Health Practitioner.

  • Knowledge of substance misuse and its effects
Key Responsibilities
  • Provide community-based engagement, assessment, and brief interventions to service users with substance misuse needs.

  • Actively outreach to the homeless in their communities.
  • Offer advice, information, advocacy, and support, providing a range of interventions tailored to individual needs.
  • Use a multidisciplinary assessment approach within safeguarding and confidentiality guidelines to build trust and increase engagement.
  • Manage a small caseload of service users with substance misuse problems, keeping them informed about available services and involved in developing recovery plans.
  • Facilitate access to clinical, psychosocial, health, housing interventions, and other community services.
