We have an exciting opportunity for an enthusiastic physiotherapist aspiring to pursue a career within frailty. This is suitable for an experienced Band 5 looking to develop skills at Band 6 level, or for an existing Band 6 physiotherapist looking to specialise.

We have a comprehensive frailty rotational pathway which balances the development of specialist knowledge and skills, with gaining a diverse experience in a range of care settings.

Job Description
Job responsibilities

The rotation covers two organisations Portsmouth Hospitals University NHS Trust and Hampshire and Isle of Wight NHS Trust, allowing you to develop skills across the whole patient pathway. While each clinical area will have a different bias, and the expectations will depend on the experience of the successful candidate, all comprise of holding a clinical caseload, developing advanced skills in frailty assessment and treatment of the older person. Within this frailty specialty you will be able to develop all of your core physiotherapy skills with complex patients while benefitting from the expertise of our team leads, clinical specialists and other members of the MDT.

The rotations are split over both employing organisations and include: Urgent Care Team (covering ED and our Acute Medical Unit), Orthogeriatrics, Acute Older Persons Medicine, Inpatient Rehabilitation, Community Physiotherapy and Urgent Community Response Team.

From the exciting, fast-paced and quick decision-making skills of admission avoidance and urgent care, to carefully planned goal-oriented and meaningful rehabilitation you can truly make a difference. You will work as part of an MDT and within integrated therapy teams; have opportunities to influence service provision within each area; as well as gaining knowledge of the wider pathway. Further to this, Portsmouth Hospitals University NHS Trust will support you to develop your clinical and leadership skills, and with the growing partnership with Portsmouth University you may have opportunities to become involved in research.

The service is covered over seven days and you will be required to work weekends and potentially different shift patterns on certain rotations. The post-holder would also be expected to work as part of the respiratory out of hours on-call rota. On-call training and competencies are provided by a core group of respiratory physiotherapists via a well-established training programme.

You must be a car driver and owner with the relevant business insurance cover and be able to use your car for work.

Key Responsibilities
  • To be professionally and legally accountable for all aspects of own work, including the management of patients in your care.
  • To undertake a comprehensive assessment of patients including those with diverse or complex presentations/multi-pathologies; use advanced clinical reasoning skills and manual assessment techniques to provide an accurate diagnosis of their condition.
  • Formulate and deliver an individual physiotherapy treatment programme based on a sound knowledge of evidence-based practice and treatment options using clinical assessment, reasoning skills and knowledge of treatment skills e.g. manual physiotherapy techniques, patient education, exercise classes, electrotherapy techniques and other alternative options.
  • To identify health and social care problems requiring assessment/intervention from other agencies, and with the consent of the patient action those referrals.
  • To take delegated responsibility from the Therapy Team Leader for managing patients with conditions specific to the specialty and be responsible for providing specialist physiotherapy assessment and treatment plans.
  • Formulate accurate prognoses and recommend best course of intervention, developing comprehensive care plans in conjunction with the patient and the multi-professional team.
  • Assess patient understanding of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack the capacity to consent to treatment.
  • Evaluate patient progress, reassess and alter treatment programmes if required.
  • Manage clinical risk within own patient case load.
  • Work within local and national guidelines. Monitor own and others quality of practice as appropriate (according to Chartered Society of Physiotherapy standards of practice).
  • When working in a domiciliary setting, manage work as a lone practitioner (with telephone support from a specialist therapist if required). This is in adherence to Trust and service home working policies.
  • To keep accurate, legible records of patient care in line with service guidelines.
  • Participate in a weekend working rota/shift working as required and the respiratory on-call rota.
