Summary:

Participate and assist in the provision of clinical pharmacy services on a rotational basis.

Liaise with pharmacy staff, medical, nursing, and other personnel to ensure safe and effective pharmaceutical care.

Actively prescribe discharge medication according to Trust protocol.

Undertake clinical and medicine management audits and implement changes for compliance.

Contribute to quality improvement programs related to pharmacy and medicine usage.

Provide medicines information service and pharmacy helpline to staff and patients.

Ensure compliance with pharmacy and medicines laws and regulations.

Collaborate with medical and nursing staff to reduce medicines wastage and ensure formulary compliance.

Participate in the drug recall system in consultation with management.
